School Physicals
Woodlawn District 209 will be offering school SPORTS PHYSICALS to students on Tuesday, May 7th. A sports physical will be required for your child to participate in sports next school year.
Please note:
- Cost is $10
- Physicals will be held at each school
- Requires a parent-signed permission form
- Requires the IESA/IHSA sports physical form
Forms can be picked up in school office and turned in prior to May 7. For more information, contact either school office.
