School Physical Form - Required Child Health Examination
Asthma Action Plan - Illinois school code now requires schools to request an Asthma Action Plan for each student with asthma. Click on the link to open the Asthma Action Plan form. The doctor managing your student's asthma can complete this form. It must be renewed at the beginning of every school year.
Food Allergy Plan - Outlines recommended treatment in case of an allergic reaction, is signed by a physician, and includes emergency contact information. It must be renewed at the beginning of every school year.
HEALTH REQUIREMENTS:
K, 2nd, 6th, & 9th Grade - Proof of Dental
K, 6th, & 9th Grade - Physical Health Examination
Religious Exemption - For Immunizations of Examinations
